Health Home Initiative - Greater Rochester Health Home Network, LLC:

The New York State Department of Health is implementing a Health Home program to provide care management for Medicaid recipients identified as having complex conditions. The Rochester Integrated Health Network, Inc. was approved by NYSDOH as a Health Home to serve Medicaid and Dual Eligible (both Medicaid and Medicare) enrollees served by it's robust provider network, including mental health, substance abuse, primary and specialty care, hospitals, and community-based organization services in Monroe County.

The Rochester Integrated Health Network formed the Greater Rochester Health Home Network LLC (GRHHN) to implement it’s vision for an effective Health Home in meeting the needs of the most vulnerable using a person-centered approach.

If your organization wishes to participate as a Health Home Network Provider, please complete the "Health Home Provider Information Form" attached and send to info@therihn.org for consideration. If your organization wishes to provide care management, please be sure to identify the number of individuals or "slots" for whom you will be able to provide care management.

If your organization wishes to participate as a Community Based Services Provider as part of the Greater Rochester Health Home Network, please complete the "Service Provider Information Form" attached and send to info@therihn.org.

Refugee Provider Subcommittee:

The Rochester Integrated Health Network Board approved the formation of a Provider Subcommittee on Refugee Care to explore options to improve refugee care and bring recommendations forward. Various potential strategies to optimize refugee care are under discussion. Please check back for more information. If you are interested in more information before then - please contact us.

 

 

Behavioral Health Access Provider Subcommittee:

A subcommittee to address concerns regarding behavioral health access was formed to understand current behavioral health access issues and make recommendations for improvement.  More information will be posted as the group begins their work. Primary Care Week:

The Rochester Integrated Health Network is pleased to host a special presentation by Dr. Neil Calman in support of the University of Rochester's Primary Care Week. Dr. Calman will be presenting "Community Health Centers - Strategy for the Next Decade" on October 24, 2011 at 6:00 pm at the Rochester Academy of Medicine. About Neil Calman, MD

Neil Calman, M.D. is a Board Certified Family Physician and co-founder of the Institute for Family Health, where he has served as President and CEO since 1983. The Institute operates 16 community health centers in the Bronx, Manhattan and Husdon Valley and eight homeless healthcare sites, as well as two school-based health centers and two family medicine residency programs. He has been practicing in the Bronx and Manhattan for the past 30 years.

Dr. Calman is the recipient of many national and regional awards for his work including the Robert Wood Johnson Foundation's Community Health Leadership Award, the American Academy of Family Physicians' Public Health Award, the Pew Charitable Trusts' Primary Care Achievement Award, and the Physician Advocacy Award form the Institute on Medicine as a Profession.
